For Team Sponsorships including:

(If you are sponsoring more than one team, you’ll need to fill out the registration form separately for each team.)

• Fill in your business name and all contact information.

• Fill in the names and contact information for all the team members that you know. If there are spots on the team for which you don’t yet have names, leave those as Unknown for both the First and Last Name.

• At the bottom of the form just above the submit button, you have the option to select and pre-purchase Team Mulligans and/or Team Skirt. These are limited to one each per team. If you don’t purchase now, you will have the opportunity to purchase this during check-in at the tournament.

• Submit the form.
